Requirements for CEU Credit

To earn CEU, you must meet ALL the following requirements:

1. Participate in the entire event.
2. By CEU DEADLINE:
   Download, complete and submit CEU Request Form to Southeast ADA Center.

CEU Request Form

1. Login to your Account or create New Account.
   Note: Each person who seeks CEU credit must have an Account; this same Account is used to register and seek applicable education credit. For questions, try Reset Help / Forgot? or contact Southeast ADA Center.
2. After successful login to your Account, select event that you seek CEU credit.
3. Select link: CEU Request Form
4. Download and complete CEU Request Form:
   • Participant, Address, Phone, & Contact
     Note: Except for SU ID Number & SSN (leave both blank).
   • Course Information (as provided per instructions)
   • Sign and date Participant Acknowledgement
5. By CEU DEADLINE:
   Submit completed CEU Request Form to Southeast ADA Center via Fax: (404) 541-9002.
6. Upon receipt, Southeast ADA Center will verify your CEU Request.
7. Your CEU Request Form will be sent for processing by University College at Syracuse University.
